New Patients

We are accepting new patients of all ages.

During initial evaluations, our care providers assess suitability for outpatient care in our office and provide possible options that may include one or more of the following; Therapy, Supplemental Nutrition, Lifestyle changes, medication or other care settings.

Our treatment philosophy is evidence based, encompassing both body and mind with emphasis on minimal use of prescription drugs.

Once you read our Office Policies and agree to them, you may call to schedule an appointment. You must abide by these policies to obtain an initial evaluation appointment and remain a patient.

To save time and avoid filling in forms in our waiting room, you may download the New Patient Forms Packet to complete and bring with you for your initial evaluation appointment.

Secure video (TeleHealth) appointments are available for return visits.
A computer, laptop, tablet or smartphone with camera, microphone & internet access are required.

To make your return appointment by secure video (TeleHealth), send an email to with “TeleHealth” as the subject line and we will reply with instructions and a link to join a secure video session with your provider. (Established patients only)

To become a new patient, gather your insurance information and call; (772) 878-7216 and select option 1.
Note: High call volume may route your call to voicemail, if this happens leave only your name and the best number to reach you. We will call you back within 24 hours. (72 if you call on Friday)

We provide automatic appointment reminders via text message, e-mail and/or automated voice response messages to your phone. Please check the blocks on your New Patient form to indicate the services you prefer. Please Note: Appointment Reminders are for your convenience and cannot be guaranteed due to the nature of the internet. Patients remain ultimately responsible for keeping their appointments even if reminders are not received or acknowledged.
